This activity is intended for community health workers.
Statement of Need
Many deaths and events of serious harm happening in U.S. healthcare are preventable. These poor health outcomes happen for several reasons. These reasons include the Social Determinants of Health (SDOH). SDOH can be a lack of access to healthy food, limited access to health care, and low income. They can also include culture, language, education, and low health literacy. Community Health Workers (CHWs) create good health and health behaviors by helping people access health resources. This course prepares you to connect people in your community with easy to understand health information.
