Statement of Need
Many deaths and events of serious harm happening in U.S. healthcare are preventable. These poor health outcomes happen for a number of reasons. These reasons include the Social Determinants of Health (SDOH). SDOH can be a lack of access to healthy food, limited access to health care, and low income. They can also include culture, language, education, and low health literacy. Community Health Workers (CHWs ) create good health and health behaviors by helping people access health resources. This course prepares you to connect people in your community with easy to understand health information.

Learning Goal and Outcome:

Learning Goal: Participants will increase knowledge about the social determinants of health and how it impacts the health outcomes in the community.

Learning Outcome  
Participants will connect the community members to the available health resources to improve their health outcomes.


Educational Objectives
Upon the completion of this activity, learners should be able to:

1. Recognize the impact of social determinants of health in the health outcomes of community members
2. Identify the health care needs of community members
3. Recognize the available health care resources in the community
